Great Lakes Integrated Network (GLIN), an affiliate of Kaleida Health and Erie County Medical Center Corporation, is a Buffalo-based, physician-led independent physicians association (IPA) consisting of approximately 600 physicians in the areas of primary and specialty care. GLIN is seeking a Manager, Post-Acute Operations to join our team.
Job Responsibilities:
- Guide the GLPA team to reach goals; both established and potential and monitor their progress
- Process weekly reports to monitor staff productivity
- Evaluate reporting of patient data
- Chart auditing to ensure accurate, high-quality care
- Assist with ( or be responsible for) all assigned projects and programs related to GLPA as it relates to staffing, coaching, interviewing, onboarding, orientation, time, and attendance.
- Act as a liaison between GLPA and GLIN network partners including providing practices, hospitals, and skilled nursing facilities
- Work with Director, Post-Acute, on corrective action issues as well as write and administer annual performance reviews
- Gain training to be named a subject matter expert on the current EMR platform and be the point of contact; intermediary between Post-Acute Team and vendor
- Deliver exemplary customer service in order to provide a positive patient experience across the organization
- Encourage open communication
- Develop department policy and procedure
- Perform other duties as assigned by management
Qualifications or Education, Training, and Experience:
- Experience (Minimum needed): 5-7 years in Healthcare Setting; 2-4 years’ experience in management in a healthcare setting
- Education: Bachelor’s degree in business, management, or related field preferred and/or an AAS RN with 5-7 years of management experience

Patient Care Navigator

Great Lakes Integrated Network's Quality Outreach team is seeking a Patient Care Navigator to serve as a liaison for patients within post-acute and home care service lines preventative and assisting in the coordination of care services. This position also facilitates gap in care closure to support our success in various quality programs as well as improving patient outcomes.
Job Responsibilities:
- Supports the providers within the service line through providing care coordination services to patients identified as appropriate for care navigation
- Works collaboratively with discharge planning at partner SNF sites to facilitate post-discharge follow-up care for patients
- Coordinates and track monthly data points for Post-Acute and Home Care Program including referral tracking and re-hospitalizations
- Works with Great Lakes Health System Care Management to facilitate assignment and initial visits for patient’s requiring primary care
- Main point of contact for the Post-Acute Direct phone line
- Participates in service line initiatives including pilot programs, weekly and monthly scorecard meetings, monthly service line meeting, and care management meetings as needed
- Works collaboratively with the Vice President of Post-Acute Services on all facility specific clinical programs
- Delivers exemplary customer service in order to provide a positive patient experience across the organization
- May perform other duties as assigned by management
Qualifications:
Education: Medical Assistant or LPN schooling or certificate required
Experience (Minimum needed):
- 2-4 years in a healthcare setting
- 3 years experience as a patient service representative
- primary care experience preferred

Social Worker- Care Management

Great Lakes Integrated Network (GLIN), is a Buffalo-based, physician-led independent physicians association (IPA) consisting of approximately 600 physicians in the areas of primary and specialty care. The mission of GLIN as a Clinically Integrated Network is to work as a community of providers to accelerate smart, patient-centered and value-based medicine, with the goal of building a healthier WNY community. GLIN is seeking a Social Worker-Care Management to join our team.
The Social Worker/ CM will work on a multidisciplinary healthcare team in a primary care/telephonic setting; focusing on coaching and coordination of care for patients needing navigation and addressing patient care needs and follow up after clinical care. The Social Worker will identify the needs of patients at risk and assist the providers to develop processes for managing the patient’s preventative care, transitions of care, and or chronic disease management using defined protocols as well as their own sound judgment.
Duties and Responsibilities (including but not limited to)
- Assess identified members to determine members appropriate for management early in their disease process and at any time during the continuum of care.
- Complete a comprehensive assessment to identify patient risk and develop a care plan utilizing expertise and judgment to evaluate needs for alternative services as needed.
- Assess members’ Social Determinants of Health, such as housing, food, transportation, and safety in the home.
- Work collaboratively with physicians and community resources including pharmacists, nurses, registered dieticians and other disciplines to address patient needs as identified in assessments.
- Assess and screen members for behavioral health concerns (depression / substance abuse) utilizing screening tools, including the PHQ2 and 9 Depression screenings, and insure they are receiving appropriate behavioral health interventions.
- Develop, facilitate, and communicate a plan of care in partnership with the member, family (or designated representatives), providers, and multidisciplinary care team to assess the options of care including use of benefits and community resources.
- Coordinate necessary referrals and authorizations pertinent to patient care and well-being.
- Utilize developed systems, processes, and initiatives to engage patients in relevant social activities necessary to promote wellness and care at the right place and time.
- Facilitate member adoption of strategies to promote physician recommended behavior changes.
- Identify and utilize cultural and community resources and align with the patient’s cultural preferences as much as possible.
- Coordinate care and communicate with multiple providers, internal and external to the practice.
- Act as a resource for both clinical and non-clinical staff [i.e. care coordinators, dieticians, RN Case Managers].
- Attend required training and collaboration sessions [i.e., learning sessions/ practice team meetings] as scheduled.
- Provide and facilitate open communication, regarding patient status, with physicians and patient care team.
- Develop constructive relationships with internal GLIN population health team members, participating providers, and community resources.
Qualifications or Education, Training and Experience
- Valid and current LCSW or LMSW licensure
- 3-5 years’ care management and/or managed care experience in one of the following settings: acute inpatient, rehabilitation, sub-acute, skilled facility, homecare, ambulatory care management, or managed health plan.
Working knowledge of the following required:
- Timely and accurate documentation of day-to-day activities in designated technology platform.
- Adaptable to new technologies and software.
- Proficiency in EMR system(s), Outlook and data entry experience preferred.
- Basic PC skills (MS Word/Outlook/PPT/Excel).
- Knowledge of Federal and State regulations for Medicare and Medicaid and other national and state funded programs.
- Knowledge of community resources access.
